I want her to be unmistakably recognizable between birth and advanced adulthood. The world is post-apocalyptic, so it wouldn't be a stretch to have any cosmetic deformities to go untreated. She needs to be able to walk around and use both hands with full arm movement. She is a mage, and never gets into combat. I was going to put her in a neck to ankle robe, but most female costumes in that world put more yardage into the drapey bits than the parts that cover anything. She does not need to be attractive, but I'm not sure if I want her to have squick factor or not. She's a minor character, so I might be able to work around something that affects her ability to speak.

I think this is going to be a particularly difficult feature to find, since these requirements basically mean she has to blend in perfectly well and yet be instantly recognizable. You might want to give up some of the stipulations to make it work; reneging on the need for both hands could allow the character to have a deformed arm with tiny, stunted fingers coming from where the elbow should be, for example.

Failing that, you could try something like a cleft lip, but that's a fairly common deformity that is both easily treatable (usually) and in a post-apocalyptic environment, probably not all that unique.

Another part of the problem comes from the fact that, apparently, magic exists in whatever world you're working with here. What sort of "mage" is she? Why can she not heal the deformity herself? Is there anyone else who conceivably could?

In short, what exactly are you going for, and does it have to be a birth defect?

She actually doesn't need to blend in, since she wouldn't be killed if she's functional enough to live. I actually started thinking about tusk-like growths or maybe an asymmetrical horn.

Cleft lip was actually my first thought, but it would have to be a fairly pronounced case since yes it likely is common. I was thinking along the lines of an extra limb, but it would be covered up by the robes and she'd probably not favor the fantasy bikini at that point.

When I say post-apocalyptic, I'm talking about Wells' Eloy and Morlocks. Actually, the latest movie adaptation is pretty much an exact copy of the world that I'm working with if you strip away Utopian appearances, right down to the existence of a time machine.

There was a problem in the past, and the king morlock didn't have the means to deal with it. This eloy girl was selected to spend her entire life learning the needed spells to give him the means, and then go back in time to burn up her remaining life energy in casting those spells. (The bulk of the story is not about her, she's just how I get there.)

Magic doesn't fix her defect because the ones training her don't care, she's too brainwashed to question anything, and besides it allows the recognition to happen. (She's carrying a letter that explains everything.)

Instead of a birth defect, you could give her a birthmark on her face. There's no need to fix it, since it's purely cosmetic. A big port-wine stain on her face would stand out, and be utterly harmless. More birthmarks here. In addition to the ones you'll find there, there is another birthmark that sometimes occurs where there is a small spot of pigmentless skin. I have one on my back, and I knew a guy who had one on the back of his head. In my case, it's just dead white skin and a bright red mole. It burns like a motherfucker in any sort of sunlight, which is annoying and painful. The guy's hair in that spot was perfectly white. If you gave here a spot like that on her face, then again, instantly recognizable and mostly harmless.

Or there is always Piebaldism, which would have few side-effects and stand out.

Ah, I seem to recall meeting someone who had irregular markings on her hands. Thank you for the tip.

I knew a girl like that. She had white streaks on her hands, and one on her head that led up into a white stripe in her otherwise-blonde hair.
